Center for Forensic Psychiatry - Replace Heated Concrete / Modify Lift Station Bar Screen / Replace BF Parking Lot

PROJECT DESCRIPTION/SERVICES REQUESTED Design services for Phases 400, 500, 600 and 700. The project will address the following items: 1. Replace existing heated concrete and all related mechanical, electrical and plumbing items for new system. Refer to yellow highlighted areas on site PDF. 2. Review and redesign the existing lift station to minimize the handling of waste materials. Provide snow redirection from roof where sidewalks are below. Highlighted cyan on site PDF. 3. Replace the existing barrier free parking lot and sidewalks at the front of the building -outlined in orange. Provide a minimum of 3 soil borings for this area. Also include all required testing during construction in your fee. o 50 and 90% review meetings for phases 400 and 500. o Cost estimate required at all review meetings. PSC will be responsible for running and documenting construction progress meetings twice monthly o DTMB will provide a site survey for the barrier free parking lot. o Heated concrete slabs are to match existing so no survey will be provided for these areas, an overall site plan will be provided. o No plans of the bar screen building exist - PSC will be responsible for documenting the building. o PDF's of the original building will be available to the awarded PSC. LEIN Check (Department of Corrections ONLY) All contractor / vendor representatives attending Preproposal Walk Through Meeting must submit a Vendor / Contractor LEIN Request form five business days prior to the meeting date (See the attached Vendor/Contractor LEIN Request Form). Send the LEIN Request form, filled and signed, by email to Daniel T. Smith at email address: smithD76@michigan.gov . The email "Subject" must include (facility name, project name, date, and time of Pre-Proposal Walk Through Meeting).
