| Description | Contractor shall provide all necessary materials, supplies, tools, equipment, transportation, licenses, permits, prevailing wages, insurance, and personnel to: provide the DSH with sallyport and pedestrian gate maintenance and incidental repairs. DSH - Metropolitan as of September 1, 2020, consist of two (2) site locations: (1) 11401 S. Bloomfield Avenue, Norwalk, CA 90650; & (2) 13200 Bloomfield Avenue. This agreement is only for the primary location at DSH - Metropolitan, 11401 S. Bloomfield Avenue. The services shall be performed for the Department of State Hospitals at the following location: DSH-Metropolitan 11401 South Bloomfield Avenue Norwalk, CA 90650 DSH-Metropolitan Alternate Care Site 13200 Bloomfield Ave. Norwalk, CA 90650 The term of any agreement resulting from this IFB is anticipated to be December 1, 2022, through June 30, 2025.
