| Description | Perform Asphalt Replacement and Repair work at the following YMHA locations: - Amedia Plaza 131 W. Boardman Street Youngstown, Ohio 44503 - Gutknecht Towers 110 E. Wood Street Youngstown, Ohio 44503 - Norton Manor 1400 Springdale Avenue Youngstown, Ohio 44505 - Vasu Manor 137 Roosevelt Drive Youngstown, Ohio 44405 - Struthers Manor 585 Poland Avenue Struthers, Ohio 44471 - Lowellville Park 810 Wood Street Lowellville, Ohio 44436 Contractor to supply all equipment, labor and material to complete the project as listed below. 1) Remove/repair areas of damaged asphalt as directed by the YMHA Development Department 2) Hot pour fill exposed cracks 1/4" wide or wider 3) Overlay asphalt with State of Ohio grade #448 Type I 2" minimum after compaction 4) Replace asphalt. Excavate min. 12"; Install 8" 304 slag; 2 1/2" 448 Type II intermediate course; 1 1/2" 448 Type I wearing course. 5) Adjust existing casting to grade (If necessary) 6) Replace existing storm inlet grate (If necessary) 7) Pavement marking as required. Contract Period: One year period, with four (4) one year options. | ||||||
