Description General: 1. Contractor is to provide all labor, tools, equipment and materials necessary to perform the full scope of work at the Salt Lake City VA Medical Center (SLCVAMC). 2. Contractor employees are to be supervised by a qualified on-site supervisor who will be responsible for employee direction, performance, appearance, conduct, quality of workmanship, safety and OSHA compliance. 3. Contractor employees shall be uniformed in apparel appropriate for the work. 4. The on-site supervisor shall be equipped with a cell phone for communication with the SLCVAMC Boiler Plant Supervisor. 5. Issues preventing the contractor from performing the full scope of duties will be reported to the SLCVAMC Boiler Plant Supervisor in writing. 6. Contractor must provide a detailed invoice, showing services rendered, date of service, hours worked, and associated costs. This information serves as evidence of compliance with contract stipulations, and is basis for payment. 7. All hazardous materials shall be handled, stored and used in accordance with applicable NFPA, OSHA and EPA standards, as appropriate for each material. 8. The contractor is to preserve and protect all structures, utilities, and equipment on or adjacent to the work site during performance of work required under this contract. Any damage caused by failure to exercise reasonable care shall be the responsibility of the contractor. Filter Replacement: 1. Contractor shall furnish and install new Siemens fire alarm relay modules, detectors and strobes in the following locations: a. GC00B-14 Two Relays (B1/B14 door holder function). b. GA00B B1 Replace door holder and add a relay. c. 1B00F B14 Add two relays (B1/B14 door holder function). d. 2B00A B1 Add one relay. e. 2A00B B1 Add one relay. f. 3A00B B1 Add two relays (B1/B14 door holder function). g. 3B00B B1 Add two relays (B1/B14 door holder function). h. Replace a bad strobe in B3 GA15Staff Restroom. i. Add a strobe in B14 2A47 womens restroom. j. Add a smoke detector in the elevator lobby in B2. 2. The Siemens relays are to be MXL-type devices. 3. The programs in the main FACP panels in B1 and B14 will be updated with the new devices. 4. Devices will all be tested for function. 5. At each location the fire alarm circuit will be tied into the nearest addressable device SLC. 6. Where devices are being added, if there is a hard lid ceiling the addition of access hatches is excluded from this quote and will be provided by the VA Medical Center. 7. Scope of work includes all materials, parts and labor for complete installation of fire alarm relay modules, strobes and detectors at all locations listed. All components shall be fully functional at completion. 8. At completion of installation, doors, walls and any other items disturbed in completing the work shall be put back into operation and tested/certified to ensure compliance with all applicable policies and regulations.
