Memorial Park House Door and Frame Replacement - Rebid

The City of Laconia seeks proposals from qualified vendors capable of providing the removal of current doors and frames and replacing them with EFCO door frames and Medium Stile Doors at the Memorial Park House. The proposal should include the following: 1. A brief description of the firm and its related experience in similar projects including appropriate summaries of projects. 2. Specification sheets of the proposed doors and frames. 3. Applicable warranty information. 4. A minimum of two (2) references, with contact information, from customers who have contracted similar services. 5. Anticipated schedule of work. 6. A comprehensive cost proposal. The City of Laconia reserves the right to accept or reject any or all proposals or any part thereof, to waive any formality in the process, and to accept the proposal considered to be in the best interest of the City. The City may cancel this proposal request at any time for any reason. Failure to submit all information called for may be sufficient grounds for disqualification. Scope of Work: 1. Removal of current doors. 2. All work should be the exact products as below or equivalent. 3. One (1) EFCO 402 door frame sized approximately 40" x 81 5/8" to include an undersized 3'0"x 6'8" EFCO T300 Medium Stile door. See example SF1 for approximate size and configuration. 4. One (1) EFCO 402 door frame size approximately 51 3/4 " x 81 5/8" to include an undersized 3'0" x 6' 8" EFCO T300 Medium Stile door. See example SF2 for approximate size and configuration. 5. Two (2) EFCO 402 door frames sized approximately 51 3/4 " x 81 5/8" to include an undersized 3'0" x 6'8" EFCO T300 Medium Stile door. See example SF3 for approximate size and configuration. Door Frame finish to be EFCO bone white. 8. Glazing to be 1" Dlear Tempered Insulated Units with Energy Advantage Low-E on the #3 surface. 9. Glazing @ lower lites to be 1" White Stucco Panel. 10. Caulking color to match. The selected firm must provide a certificate of insurance satisfactory to the City of Laconia (see attached Insurance Requirements form). After receipt of proposals, the City will evaluate the proposals and will use the following criteria in its evaluation: 1. Qualifications of the vendors. Considered in this category will be the: a. Relevant experience of the vendor in installations. b. Reputation of the firm based on references. 2. Technical approach of the proposal to include responsiveness to meet or exceed the requirements of this proposal request. 3. Proposed door, frame, hardware, etc. 4. Proposed cost.
