Description

Eagle living Center landscaping project 666-22-112 for plants, Evergreen trees, medium and small trees, sod, mulch, and weed matting. Contract Period: Base POP Begin: 05-08-2023 POP End: 08-08-2023 PRINCIPAL NAICS CODE: 561730 - Landscaping Services PRODUCT/SERVICE CODE: S208 - Housekeeping - Landscaping/Groundskeeping SCOPE OF WORK (SOW) Eagle Living Center Landscaping 666-22-112 Veterans Affairs (VA) Health Care System, Sheridan, WY Statement of Work Introduction: The Sheridan Veterans Affairs Health Care System (VAHCS), located at 1898 Fort Road, Sheridan, WY 82801, requires professional landscaping of the grounds around their Eagle Living Center (ELC). Construction of the ELC patient unit, an addition to Building 86, was finished in early 2020. This requirement is to complete the landscaping around the ELC, according to the project's landscaping plans, including, but not limited to, planting flowers, plants, trees, installing sod, and mulch. Professional landscaping services are needed to give these grounds a hospitable and finished appearance on our station. Irrigation systems have already been installed on these grounds, are currently operational, and are not part of this scope of work. Statement of Work: 1. General a. Construction project is to be completed within 90-calendar days from date of Notice to Proceed. b. Contractor shall provide all equipment, materials, tools, supplies, and labor required for work under this SOW, providing landscaping services as indicated in the plans, drawings, and specifications per VA construction standards. c. The Contractor shall field verify all site conditions, dimensions, and parameters prior to beginning work and notify the Contracting Officer's Representative (COR) of any discrepancies. d. Safety submittals shall be completed and approved prior to the commencement of on-site construction work. Material-selection submittals--including tree, plant, and landscaping material submittals--shall be completed and approved by the COR prior to ordering of materials for the project. e. The building tenants, including staff, patients, and visitors, not in the construction areas will remain at work in the buildings during this construction. The Contractor shall make patient safety and infection control measures a priority. The Contractor shall coordinate work locations and schedules with the COR to minimize impact to building tenants. f. All landscaping services, earthwork, product design, project management, and other work specified in the SOW, plans, drawings, and specifications for this project shall be provided by the Contractor. g. The installation must, as a minimum, be in accordance with all applicable codes indicated in the Department of Veterans Affairs Design Manuals and VA PUBLICATIONS (Master Construction Specifications, Construction Standards, National Fire Protection Association (NFPA) Underwriters Laboratory, International Building Code IBC 2003, NFPA and National Electric Code (NEC) building codes and standards and VA Information Security Handbook 6500.6, 6550.0 etc.). h. The Contractor shall inquire of the COR if any property removed is desired to be retained by the VA for reuse elsewhere within the medical center, at the time of removal. i. If the VA desires to retain an item, the Contractor shall deliver it to a location designated by the COR. ii. If the VA does not desire to retain an item, the Contractor shall dispose of it appropriately off-site. i. The new components installed shall be tested before VA acceptance. All testing parameters shall be provided by the Contractor with source references to the COR and 3rd-party reviewer before testing begins. j. At the conclusion of the project, the Contractor shall provide the COR with a red-lined set of construction drawings, showing any as-built variations from the plans as called for in the specifications. k. The Contractor shall repair any disturbed earth around and in the construction areas. This shall include: i. Removing any rocks or clumps that have been brought to the surface. ii. Removing any rocks or clumps larger than 1 inch in diameter from the top 3 inches of soil. iii. Filling in any ruts or depressions with approved fill material, such that the areas are level or at the required slope. iv. Returning the surface cover to what is was before the project, or as called out in the project drawings. v. In the case of grass, the contractor shall provide topsoil as necessary to provide a good medium for grass growth. If the area(s) cannot be covered by the VA sprinkler system, then the Contractor shall provide for the watering of the new grass until it is established. Any areas that the grass does not take to shall be reseeded under the warranty period. l. The Contractor shall replace any trees, shrubs, or other plants damaged or destroyed during construction, per the project landscape drawings. m. Contractor shall appropriately install stakes with posts, per the drawings, plus fencing around each newly planted tree to protect the young trees from damage caused by deer. Contractor shall maintain fencing in good repair for a minimum of two years after planting. Temporary snow fencing shall not be installed around the trees. The COR shall provide final determination on appropriate tree protection. n. The Contractor must plant the new trees and plants when the irrigation is on--between May 1st and September 30th. o. The Contractor shall receive and store all deliveries of materials and equipment off-site at their expense. Deliveries shall not be sent to the VA Medical Center nor brought on-site until they are ready to be used/implemented. 2. Safety and Security a. The Contractor shall follow all safety specifications and shall always have an OSHA 30-hour construction safety certified person on-site, at each work location, overseeing the construction. All other contractors on-site shall be OSHA 10-hour certified at a minimum. b. Work will be on the grounds of an active medical center with staff, patients, and visitors present, so the Contractor shall plan for infection control measures to include dust containment, tool/equipment control, access control in construction areas, and cleanup to hospital standards. i. The Contractor shall, with the participation of the COR and Infection Control staff, complete an Infection Control Risk Assessment (ICRA), using the VA forms and process, and shall implement all protection measures called for in the ICRA. Work will include: Base Bid: All work listed in this scope of work (SOW), the drawings, and specifications. Fees for this project shall be a lump sum. There are no deductive bid alternates.
