Description

Checks are to be made payable to Thomas Fok & Associates, Inc. Project includes partial depth asphalt removal and replacement in the most distressed locations (5%) and then a 2" asphalt pavement overlay following ODOT specifications. Approximate pavement dimensions are 2,338 ' long x 22 ' wide x 2" thick. Domestic Steel use requirements as specified in Section 153.011 of the Revised Code apply to this project. Copies of Section 153.011 of the Revised Code can be obtained from any of the offices of the Department of Administrative Services. Contractor and all Sub-contractors for labor performed and materials furnished in connection with the project involved. Milton Township reserves the right to reject any or all bids and waive any informalities in bidding. Bids of Corporations not chartered in Ohio must be accompanied by proper certification that such corporation is authorized to do business in Ohio. The successful bidder will be required to pay at least minimum wage rates pre-determined by the Secretary of Labor for the project work as required. The Schedule of Approved Wage Rates, as pre-determined by the Secretary of Labor is incorporated in the Contract Documents. This Contract is subject to the provisions of Executive Order 11246 of September 24, 1965, as pertains to an Affirmative Action Plan, and Governor Executive Order of January 27, 1972. A bidder/contractor will be deemed committed to the goals of Milton Township Plan Bid Conditions by submitting a properly signed bid.
