Description

Scope The Contractor shall provide custodial services and perform tasks as described in this Exhibit A (the "Custodial Services") at the at the following two (2) Client Agency Sites (collectively or individually as the context requires, the "Facility"): A. DAS Bureau of Property and Facilities Management Services located at 505 Hudson Street, Hartford, Connecticut with an estimated 157,000 square feet, located on ten (10) floors; and B. DAS Bureau of Property and Facilities Management Services located at 110 Sherman Street, Hartford, Connecticut with an estimated 32,170 square feet, located on five (5) floors. The Client Agency shall designate or assign a representative(s) to act on behalf of the Client Agency regarding all matters affecting the Custodial Services (the "Client Agency Designee"). The Client Agency shall identify the Client Agency Designee upon commencement of the Contract. 2. Custodial Services Contractor shall: A. Thoroughly complete the Custodial Services in accordance with the Contract, in a professional manner, using quality equipment and materials that conform to all current OSHA standards, and Federal, State statutes, regulations and requirements. B. Oversee that the Custodial Services are completed in accordance with the Contract. C. Ensure the Custodial Services conform with the "Tasks Procedure" section of this Exhibit A to ensure the Facility is uniformly clean, hygienic. D. Provide all labor, materials and equipment necessary for cleaning and maintenance, unless otherwise specified herein. E. Have a supervisor monitor its employees' arrival and departure or utilize a remote timekeeping system for monitoring employee real-time arrival and departure from the Facility. F. Provide backup staff in the event the primary cleaner(s) or the supervisor is unable to Perform the Custodial Services. Backup staff shall arrive no later than one (1) hour after the scheduled start time. G. 505 Hudson Street: Provide the Custodial Services five (5) days per week, Monday through Friday between the hours of 4:00pm and 10:00pm. The work schedule is attached as Attachment 1 Task Schedule Checklist - 505 Hudson. The scheduled tasks are based on a twelve (12) month period, identifying, and delineating the time frames, such as daily, bi-weekly, weekly, bi-monthly, monthly, and quarterly for the required Custodial Services. The Contractor shall prepare a monthly maintenance/cleaning calendar reflecting the dates for the daily, bi-weekly, weekly, bi-monthly, monthly, quarterly, and bi-annual tasks as indicated in the Attachment 1 - Task Schedule Checklist - 505 Hudson. The Contractor shall provide the Monthly Maintenance/Cleaning Calendar to the Client Agency Designee prior to the start of each month. H. 110 Sherman Street: 1. Provide the Custodial Services five (5) days per week, Monday through Friday between the hours of 5:00pm and 9:00pm. The work schedule is attached as Attachment 3 Task Schedule Checklist - 110 Sherman. The scheduled tasks are based on a twelve (12) month period, identifying, and delineating the time frames, such as daily, three times a week, biweekly, weekly, bi-monthly, monthly, and quarterly tasks for the required Custodial Services. The Contractor shall prepare a monthly maintenance/cleaning calendar reflecting the dates for the three time a week, bi-weekly, weekly, bi-monthly, monthly, and quarterly tasks as indicated in the Attachment 3 - Task Schedule Checklist - 110 Sherman. The Contractor shall provide the Monthly Maintenance/Cleaning Calendar to the Client Agency Designee prior to the start of each month. 2. Provide one (1) porter for disinfecting duties, one (1) hour per day, Monday through Friday from 5:00pm through 6:00pm, unless otherwise requested in writing by the Client Agency. Duties shall include disinfecting all high touch areas in the Facility including but not limited to doorknobs, handles, railings, entrances, elevator buttons, service tabletops, bathroom touch knobs, sanitizer pump stations and water dispenser buttons) once during the evening cleaning. The Contractor shall document with sign-in, and sign-out sheets or other documents as preapproved by the Client Agency, each hour(s) of day porter services each month and such documentation shall be included with the invoice in which the hourly rate is being billed. Contractor must remain certified as a Qualified Partnership per Conn Gen. Stat. Section 4a-82 during the entire Term of the Contract. The certification of Qualified Partnerships is administered through the CT Community Nonprofit Alliance, Inc. which can be reached directly at (860) 257-7909 for program requirements. This Solicitation contains a mandatory pre-Solicitation meeting/site-visit requirement. Contractors who are interested in responding to this Solicitation must attend the meeting/site visit at the specified date/location listed below. Late Arrivals (10 minutes or more) will not be given credit for attendance nor allowed to participate in the Solicitation process. Vendors will not be admitted to state buildings without a valid photo ID. Failure to attend this meeting will result in the rejection of your response.
