Description

**As of September 8, 2023, project was awarded, They have begun a portion of the project already with final completion anticipated sometime in October.** Work shall include but not be limited to: a. Removing existing asphalt from bus garage parking lot. Grade and compact sub-base. Install base and surface asphalt, minimum of 4" finished. b. Repair high school parking lot as needed, sealcoat entire parking lot c. Restripe parking lots Site will be available as soon as possible. Sequencing of work will be coordinated between the Owner and the contractor. The building staff will be responsible for the removal of furniture and fixtures that are in the work area. Work must be completed by August 14th, 2023 or negotiated date pending supplies and contractor availability. Bonding: For all proposals in of $50,000 or more. a. Security may be certified check, bank draft irrevocable bank letter or credit or surety bond. b. All bidders must be bondable and shall include in the Proposal the cost of furnishing Labor and Material Payment and Performance bonds each in the payable amount of 100% of the proposed bidding sum. The bonds shall hold Quincy Community Schools as the oblige. A surety authorized by the State of Michigan must issue bonds. The surety must be listed on the current Federal Register or have an A. M. Best rating of B+ or better. Failure to meet these requirements will be cause for rejection of the bond. 3.0 - SCOPE OF WORK: 3.1 Bus Garage Parking Lot (approx. 28,000 sq. ft.) - remove asphalt. Clean, prepare sub-base aggregate. Install hot mix base and surface at a minimum of 4" average compacted thickness. 3.2 High School Parking Lot (approx. 113,000 sq. ft.) - remove & replace asphalt in portion of drive entering parking lot. Fill major cracks. Sealcoat entire parking lot. 3.3 Restripe existing pavement markings. 3.4 Proof of Workers' Compensation and General Liability insurance is required. 3.5 Contractorshall provide items of a minor nature notspecifically listed to provide a complete Owner acceptable project. 3.6 (Optional) Entire District (approx.. 170,000 sq. ft.) Fill major cracks. Sealcoat entire parking lot. Restripe where necessary. DISCLOSURE STATEMENT - FAMILIAL RELATIONSHIP Pursuant to MCL 380.1267, a sworn and notarized statement disclosing any familial relationship that exists between the owner and any employee of the vendor/contractor and any member of the Quincy Community Schools Board of Education or the Quincy Community Schools Superintendent must be accompanied with the bid/proposal. Bids and Proposals without this disclosure statement will not be accepted. The members of the Quincy Community Schools Board of Education are: Al McClellan, Greg Richer, Tim Miner, Brian Preston, Erin Veysey, Shawnna Craig, and Tim Miller. The Quincy Community Schools Superintendent is Marc Kramer. The undersigned, the owner and or authorized officer of the below named contractor (the Contractor), pursuant to the compliance certification requirement provided in the Quincy Community Schools' (the "School District") Request For Middle School Science Curriculum and Mailing Request for Proposal (the "RFP"), hereby certifies, represents and warrants that the Contractor (including its officers, directors and employees) is not an "Iran linked business" within the meaning of the Iran Economics Sanctions Act, Michigan Public Act No. 517 of 2012 (the "Act"), and that in the event Contractor is awarded a contract as a result of the aforementioned RFP, the Contractor will not become an "Iran linked business" at any time during the course of performing the Work or any services under the contract. The Contractor further acknowledges that any person who is found to have submitted a false certification is responsible for a civil penalty of not more than $250,000.00 or 2 times the amount of the contract or proposed contract for which the false certification was made, whichever is greater, the cost of the School District's investigation, and reasonable attorney fees, in addition to the fine. Moreover, any person who submitted a false certification shall be ineligible to bid on a request for proposal for three (3) years from the date that it is determined that the person has submitted the false certification.
